OBJECTIVES. Immediate loading in the upper jaw has been considered a risk for a long time. The authors analyze factors influencing prognosis of immediately loaded implants and report their experience in extensive prosthetic rehabilitation of upper dental arch using an immediate loading approach. MATERIALS AND METHODS. The Authors follow a protocol based on the following guiding principles: 1) using a number of implants as close as possible to the number of natural teeth; 2) researching primary stability; 3) obtaining immediate implant splint with a temporary acrylic bridge positioned in correct occlusion. RESULTS. By analyzing clinical aspects, the Authors conelude that immediate loading not only leads to higher treatment acceptance by patients but also to a better site response. CONCLUSIONS. Bone and soft tissue show a good reaction to immediate loading with excellent osteointegration in terms of both tissue structure and quality.
